Staff Nurse III for High Risk Patients

The Staff Nurse Level III (for High Risk patients) provides nursing services at the top of his/her license to patients in accordance with all applicable Massachusetts State Laws as well as regulations promulgated by the Department of Public Health and Harbor Health Services, Inc. Duties include but are not limited to: triage; access; case management; and nursing support of clinical sessions. Staff Nurse Level III Essential Functions include but are not limited to: • Administer medications and vaccinations to patients as ordered by provider or following standing orders policy. • Assessing information about the patients, physiological, psychological, sociological, and spiritual status. • Able to conduct procedures according to policy while ensuring patient safety. •Actively participates in the manual and automated clinical tracking programs to ensure compliance with clinical outcome measures, prevention and follow-up care. •As required and/or as appropriate provides health care and disease management education as part of regular visits. •Assists in the orientation and evaluation of new staff to the department. •Health Educator for Chronic Disease population. •Ensures HHSI High Risk Patient population (OBAT, Infectious disease, Complex Care) is properly managed including, referrals, consults, tests, medication management, and patient education to ensure or move toward wellness. •Site Manager for Vaccine Coordination under the Vaccines for Children program. •Health Educator for Complex Chronic Disease population (HIV, Hep C etc.) •Complete at least 16-20 scheduled patients facing clinical hours per week of individual or group visits. Minimum Qualifications: 1-3 years of experience in medical case management in acute care, ambulatory care or health insurance environment, strong analytical and clinical problem solving skills, strong oral and written communication skills, experience in using clinical data and developing interventions, experience with Microsoft Office application particularly MS Outlook, MS Word and Excel, EMR experience. Current MA RN license and CPR/BLS required, Bilingual a plus. Addiction experience highly desired.
